| At the time of registration, parents or legal guardian must provide the
following documents. Students who move during the school year must resubmit
residental verification.
1. Withdrawal form from last school
2. Birth Certificate
3. Child's Social Security Card
4. Up to date Alabama Blue Immunization Form
5. Proof of residence (must provide any two of the following items)
** Property tax records, indicating homestead exemption
** Mortgage documents or property deed
--warranty deed filed with probate judge that has seal and recording
date
--mortgage letter that states the name and address of the
parent/guardian
--settlement statement which is a contract that is given at the time
of closing and is signed by both parties
**Residential (apartment or home) lease
--lease must be a typed and signed document that is legally binding
for both parties (not a handwritten or a notarized statement)
--a lease may include utilities if it is specified in the lease (this
type of lease is usually given to a military family or a mobile
home renter)
**Utilities (only one will be accepted)
--gas, water, or power bills are the only acceptable items
**Automobile registration
--must be current (Probate Office will not change a person's address
until renewal time unless the person has a copy of a curent utility
bill)
6. ID picture
**Any parent enrolling or withdrawing a student must provide a picture ID
