The Forms Packet includes the following forms that need to be submitted for each participant in any Trinity Athletic Program:

The 8th grade is selling Valentine Candy Grams to support their 8th grade trip in May to Catalina Island. There are several categories that you can choose from to send to someone special. Parents are encouraged to use this form to send a secret gram to their children. Students can use this form to purchase candy grams in advance for their friends and teachers.
Each candy gram is $1, Cash Preferred. There are two different choices: Hershey bar or Skittles. Candy gram orders will be accepted through Wednesday, February 8th. Please turn in all orders to your child’s teacher or to Ms. Stadler in room 603, not to the school office. Thank you for supporting the students trip to Catalina Island. and will be delivered Monday, February 13th and Tuesday, February 14thChina Red Cafe - Spirit Night Flyer (good anytime throughout the school year)Bring the flyer (click the restaurant name to open) ANYTIME you go to China Red Cafe and they'll donate 20% back to Trinity TEAM. China Red Cafe is in the Albertson's shopping center on the NE corner of Dysart and Indian School.

Please complete this form online and save a copy to your computer. This will allow you to easily make changes as needed in the future. Please include: complete addresses and phone numbers for mother & father, including state and zip code. You need to include two authorized pick up individuals (other than mother or father), and Health Care Provider (physician, physician assistant or registered nurse practitioner) name and phone number.
Any missing information will delay processing. Be sure to print, sign and date the 2nd page. You can digitally sign the form by creating a secure digital signature. Once completed you can scan and email the completed form to Lisa Stipp at lstipp@trinitylcs.org

Yearbook Order Form - updated 8/4/11
Available by advance order only. All orders must be received by Friday, 2/17/12
Sale Price: $20.00 for orders placed by 11/18/11. Orders received after 11/18/11 through 2/17/12 are $25.00. Please submit one order form for each student (you may make one combined payment per family). Forms and payments should be submitted to Lisa Stipp in the school office.
