Enrollment Forms

Enrollment packets include three forms with a total of six pages. In an effort to reduce duplicated information on multiple forms, the Enrollment Application does not require address or phone information since that is provided on the Emergency Card. 
   On the Enrollment Application include: Ethnicity, baptism date, employer and e-mail addresses.
On the Emergency Card include: complete addresses and phone numbers for mother & father, including state and zip code.  You need to include two authorized pick up individuals (other than mother or father), and Health Care Provider (physician, physician assistant or registered nurse practitioner) name and phone number.

Forms may be completed online. Any missing information will delay processing of enrollment.


New Students - a current immunization record and birth certificate need to be submitted with the enrollment forms and any required fees.

            2012 Preschool Summer Financial Agreement
            There will be two summer preschool sessions. The June session will be held 6/4 through 6/29.
            The July session will be held 7/2 through 7/27 (no school on 7/4). A $15.00 registration fee
            applies for each session. You can enroll for either or both sessions. Registration deadline is
            Friday, May 11, 2012.

Preschool class times are 8 am to 11 am or 12 noon to 3 pm

5 Day (Full day Mon-Fri) for Pre-K students only

(Tuition for the full day class includes childcare from 8am-3pm on all days childcare is needed when school is not in session and child care is open during the regular school year. Parent will be responsible for any childcare needed 6am-8am and 3pm-6pm, whether or not school is in session.)
