Parent Questionnaire


Dear Parents,

During the past week I have started to get to know your child
here in the classroom. My goal is to provide each child a year of
successful development in confidence, self-esteem, maturity,
friendship and academic knowledge and skills. Working in partnership
with parents is critical in achieving these goals. Since you know
your child better than anyone, I welcome your insight, and I want to
give you this chance to tell me anything that you think is important
as I work with your child this year. Please address as few or as
many items below as you feel necessary. Also, I hope that you’ll
feel free to mention other things that come to mind as the year goes
on. I’m always interested. As a reminder, e-mail is a great way to
communicate questions, comments and/or concerns to me. My school

    best way to reach you by phone?

  • 2) What does your child enjoy? What kinds of things does he/she choose
    to do in his/her spare time? Interests? Hobbies? Fascinations?

  • 3) Does your child have any special friends? How does your child get
    along with other children?

  • 4) What are your child’s strength’s (both academic and non-academic)?

  • 5) What are your child’s needs (both academic and non-academic)?
  • 6) With whom does your child live? Are there other children in your
    family? How do they get along?

  • 7) What are your child’s chore’s/responsibility at home?
  • 8) Are there any special considerations I should be aware of this year?
    (This could range from allergies, exciting plans, new siblings, a
    recent move etc.)

  • 9) How does your child feel about past learning situations?
  • 10) What do you consider to be most important for your child this year?
    (What are your priorities?)
