Link for Tdap Verification Form and Information All students entering 7th and 8th grade next year must provide documentation of current Tdap immunization before they may enter school in the fall. Over 3 million students statewide will need this documentation, so avoid the last minute August rush and bring your verification form to the school office before the end of the school year. Unfortunately, students who do not have documentation of Tdap established at the school site cannot be given homeroom assignments or class schedules in August.
