Dear Students, Parents and Guardians:

Welcome to Team 6E of Glades Middle School! We look forward to an exciting and successful year. The success of your child's learning depends greatly on the teacher-student-parent cooperation. Certain procedures and regulations are necessary to ensure that your child learns and profits from the team.

During the school year students will have access to a variety of resources including computers, academic websites and curriculum-based videos. These additional resources help to create an interactive classroom environment that enhances the learning experience. Your review of this letter and returned signature page acknowledges your awareness of the use of these resources and provides permission for student participation.

Kindly complete the personal data page as well. Also, please remember to update me if there are any changes with your personal data information; such as a new telephone number, email, etc. Please return this form to your child's first period teacher.

We are looking forward to an enjoyable and productive school year. Thank you in advance for your support and cooperation. Please familiarize yourself with the school calendar so you are aware of important dates. Feel free to contact any of us about any concerns that you may have. We are confident that by working together we will have a successful year!

We may be contacted through the Glades Middle school website via the Dept. tab. The assignments, homework, and important information will be posted on each of our websites. Messages can also be left on our voicemail and we will return your call as soon as possible. All conferences are team conferences. Kindly schedule them through your first period teacher.

Sincerely,

Ms. Wagner (Math) Mr. VanLandegend, M.S.Ed (Science)

Mrs. Riley (Social Studies) Ms. J. Suarez MBA (Language Arts)

Folded, torn, or defaced agendas are not permitted. Please check the agenda on a nightly basis and sign the agenda every Thursday evening. Agendas will be checked periodically for a grade on Fridays.

Class Period

Novel: A novel (chapter book) is required for leisure reading. A novel must be with their book bag at all times. Students may check out novels from the media center before school and/or visit your local public library.

Grades: Student grades are posted regularly to Pinnacle and may be checked at any time by both parents and students. Pinnacle can be accessed through the Glades Middle School website (www.gladesmiddle.org) by clicking on the "Pinnacle" link. To open Pinnacle, the student ID number (located on the back of the student ID Card) and student birth date are required. Interims and report cards will be sent home each quarter to inform parent of grades.

Dress Code: A unified dress code is to be followed by all students at Glades Middle School and will be strictly enforced. Please see the Glades Middle School website for a full listing of uniform items.

Textbooks: Textbooks in all subject areas will be loaned to students for the academic year. Students are required to pay for lost or damaged books. Textbooks dues will follow the student to high school and will delay high school graduation.

Chaperone/Volunteer:

We have many exciting plans for fun and educational field trips for our students, but cannot do it without your help. We are planning about 4 field trips this year, and need 5-10 chaperones for each trip. Due to recent legislature changes, The Jessica Lunsford Act was implemented and states that "...persons who are permitted access on school grounds when students are present and who have direct contact with students must meet Level 2 screening requirements." Chaperones must pass a security background check each year that can be completed through http://www.getinvolvedineducation.com/. It takes about one to two weeks to be cleared. Please feel free to call Coral Dominguez with any question or to check your clearance at 754 323-4613

Keep this letter for your records; please do not return this portion.

Please sign and return to: Mr. VanLandegend.
