Kindergarten Registration for 2011-2012 School Year (or any student new to Springwood.)

KINDERGARTEN REGISTRATION IS GOING ON AT SPRINGWOOD ELEMENTARY FOR THE 2011-2012 SCHOOL YEAR
IN ADDITION TO FILLING OUT THE REGISTRATION PACKET THAT YOU MAY PICK UP AT THE FRONT OFFICE, YOU WILL ALSO NEED TO PROVIDE THE FOLLOWING DOCUMENTATION:
• Certified birth certificate (not hospital copy) we will make copy here
• Current health physical, completed after August 2009 (Physician provides this form.)
• Immunization record (Physician provides this form.)
• 5 DTaP (diphtheria, tetanus, acellular pertussus)
• 4 Polio
• 2 MMR (measles,mumps,rubella)
• 3 Hepatitis B
• 2 Varicella (chickenpox)
• Two proofs of address --homestead exemption card, mortgage payment book, lease agreement, house insurance, voter's registration card, or most recent utility bill are acceptable. When you come to register your child please bring all your documentation with you. Please make sure to complete the registration process by August 2011.
