Information for registering in the public school system for the purpose of participating in the Private School Tax Credit program.
 
Dear Elaine:
 
I spoke with Sue Garrison from Registration for the FC BOE today.  She is fully aware of the law and has received direction of the GA DOE.  Parents looking to register their children in order to qualify them for potential scholarships through an SSO need to follow the BOE's standard procedure and advise them that they do not intend to have their child attend their district's school.  They will issue a letter advising that child is enrolled, but did not show up or something like that.
 
Children who have previously been enrolled in the FC School do not need to update their medical and dental/eye forms.  They will accept the ones on file from when the child was enrolled.  This is great because Hali attended a FC School for two years prior to Cornerstone so her docs are on file.  Any parents who do not have updated forms and do not want to obtain them, can make an appointment with the FC BOE Nurse.  I do not know anymore about the procedure with her.
 
The following documents are required to register a child a FC School.  Registration is taking place at their Elm Street facility in Cumming.  Obviously, parents would have 30 plus days to register their children in public school pending approval from the Dept. of Revenue regarding their tax credit application.
 
1.  Two proofs of residence - lease, deed, tax bill and current utility bill;
2.  Proof of DOB - certified birth cert., passport, etc.;
3.  Immunization form 3231;
4.  Eye, ear and dental form 3300;
5.  Original or copy of Social Security card;
6.  Withdrawal form, records or report card from other school.
