ATTENTION 5th GRADE PARENTS!
PLEASE TAKE NOTE AND THINK AHEAD!
Students entering the sixth grade must have a certificate showing proof of
two measles, two mumps and rubella vaccinations administered since the first
birthday. Sixth graders must also have a certificate for 2 varicella
(chicken
pox) vaccinations or history of disease or blood test and date by the health
care provider on form 3231.
