Students entering Kindergarten in the fall will need a physical exam
which can be dated within 1 year prior to the start date of school to be
valid as well as up to date immunizations.
As required by the Illinois Department of Health, all physical exams are to
be on the 2 sided form. If you are in need of a form, it can be
downloaded and printed from the District 20 website under District Health
Services.
Parents are to complete, sign, and date the health history portion on the top
of page 2. Make sure your physican has completed the entire form including
height, weight,the diabetes screening portion including BMI percentage, and
lead screening and has signed and dated all appropriate areas. All the above
areas are required for the physical to be considered in completion.
All Kindergarten and 2nd grade students are required to have a dental exam
completed and on file at school.
All Kindergarten students or students enrolling for the first time in an
Illinois school are required to have an eye exam completed and on file at
school.
Students can get their required immunizations, physicals, dental exams,
and eye exams done now and the "summer rush" that health care provider's
offices experience can be avoided!
If you have the physical or dental exam done prior to the end of this
school year, bring the documentation to the school health office anytime
during school hours.
