2012-2013  Information Packet
Please find in your 2012-2013 packets the following:

          1.     Directory Questionnaire/Email Questionnaire (online)
                  Directory & Email Questionnaire Online Form
                  Must be entered before May 28th, 2012.
          2.     Emergency Cards (one per family/fill out both top & bottom-
                  back and front-Make sure to sign back side)  One card
                  stays in the school office and the other goes to the health
                  office.  Turn in hard copy to office by May 28th, 2012.
          3.     Health Office Medical Packet (also available online in case
                  hard copy is lost).  Must be returned by August 15, 2012.

          5.     Terra Nova Test Results (Grades 3 through 7)
          6.     Holy Cross School Supply Lists (online)
