Physical Forms are available in the Forms/Documents section of the website. All students must have a completed KHSAA physical, JCPS Addendum to Physical form, purchase student athletic insurance, and view the Health and Sports Safety video prior to participation. In addition, one parent must view the Health and Safety video prior to a student being eligible to participate.
