When does my elementary school-age child need a new physical exam?

Students entering the Foxborough School System for the first 
time,
kindergarten, and grade four are the times a new physical exam is
needed for
your child.

What immunizations are required for my elementary school-age child?

The following immunizations, mandated by the Massachusetts 
Department of
Public Health, are required of all students entering the
Foxborough School
System: D.P.T. - 5 doses
Polio - 4 doses
M.M.R - 2 doses (first dose after age 12 months)
Hepatitis B - 3 doses (dose 1-2 with 30 days between,
dose 2-3 with
85 days between, dose 3 is after age 6
months)
Varicella - 2 doses after age 12 months, starting September 1, 2011, or a doctor
certified history of month and year of chiken pox disease.

Does my child need a lead screening before entering school?

Yes, your child is required to have a lead screening done by the 
physician
before entering kindergarten, per the Massachusetts Department of
Public
Health.

What does the new Massachusetts Pre-Kindergarten screen involve?

The Massachusetts Pre-Kindergarten screen is a tool started in 
2006. This
involves an acuity and stereopsis test, to be performed by your
child's
pediatrician. If your child fails, a referral will be made to an
ophthalmologist for furthering screening. This mandate is being
used for
early detection of visual problems and interventions.
