members. Links to the surveys will be made available at the end of each
school year. Please revisit this page in June 2010 to contribute your
valuable input regarding the Title One Program.

1. Title 1 Needs Assessment Survey:
The Title 1 Needs Assessment survey is given to parents, teachers,
administrators, school councils, and community members in regards to the
grade level, subject area, and delivery method for our program. This survey
has 5 questions and should take approximately 2 minutes. Questions 1-4 are
mandatory. This information is part of the overall collection that guides us
in developing our program for next year.

2. Title 1 Parent Involvement Program Survey
The Title 1 Parent Involvement Program Survey is given to Title 1 parents.
The federal guidelines for Title I include conducting an annual parent
involvement program review. This yearly survey will be used to gain insight
to help us evaluate the effectiveness of the Title I Parent Involvement
program and services at the Placentino School.
The survey consists of 20 questions. It should take no more than 15 minutes
to fill out. The questions will accept only one answer. The comment section
at the end gives you an opportunity to share your thoughts and suggestions
with us. Your input will help us provide the services to help your student
be successful in school.
