In order to give the best care possible to your child, please keep me
informed of any new or ongoing health concerns including: severe allergies,
asthma, ADHD, diabetes, or difficulty with mobility. This will allow me to
plan any necessary accommodations your child may need. All medical
information is confidential and will be shared with school staff, only with
your permission, and on a need to know basis.
Do you need health insurance or help paying for it?
Massachusetts Department of Public Health will provide health insurance for
children under age 19. How much a family contributes toward the premium is
based on the family size and income. For eligibility requirements, call 1-
