Student’s Name:_________________________________Teacher:_____________________

Technology tools such as computers, the Internet, digital cameras, and video
production equipment offer exciting learning opportunities for East Metro
Integration District 6067 students. They also raise issues concerning
appropriate student use of these tools. In order to assure that students use
technology in a safe and meaningful way, we require that ALL East
Metro Integration District 6067 technology users follow these guidelines.
Student computer accounts will not be activated until this form has been
signed and returned to each student’s teacher.

Respect others, yourself, and equipment. This means...
• Use only your own login name and password. Keep your password to yourself.
• Make changes to your own files only.
• Be gentle with all computers and other technology equipment.
• Report any damaged equipment immediately.
• Use appropriate language at all times.

Act in a safe manner. This means...
• Keep all personal information private. When using the Internet, never
share ANYONE'S last name, address, or telephone number.
• Use only your first name and the name and city of the school when
communicating on the Web.
• Visit appropriate web sites only. A staff member must supervise student
Internet use.

Learning comes first. This means...
• Computers and other technological equipment are learning tools.
• Recreational use of technology should never interfere with somebody's
learning.
• Instructional staff will decide when Internet use is appropriate.
• Chat rooms should be used for instructional use only.

I have read and understood the school district guidelines relating to safety
and acceptable use of the school district technology system and agree to
abide by them. I further understand that should I commit any violation, my
access privileges may be revoked, school disciplinary action may be taken,
and/or appropriate legal action may be taken.
