Request a Transcript
Students who are at least 18 years old may request a transcript. Students below the age of 18 must have the request form signed by a parent or guardian. The form below may be used to request a transcript by submitting the form to the guidance records clerk or by mailing it to 401 Arrow Drive, Clinton, MS 39056. Trancripts will not be faxed.
