•Use of putting green and practice range
•Steak banquet immediately following golf
•There will be cash beverage carts on the course
Shot gun start at 1:00. Please try to arrive 45 minutes
before to sign in.

Hole Sponsor Registration Form
The PTA from The Mayo School, in Holden will be holding a golf tournament on
Sunday, June 22, 2008, at the Holden Hills Country Club. Please consider
becoming a sponsor, as your donation is tax deductible. All proceeds for this
tournament will be used to support the many programs of the Mayo PTA. These
programs include field trip transportation, student enrichments, educational
aids, library books, playground & gym equipment and much more.
In order to make our golf tournament a success we are in need of your
donation. When your business sponsors a hole, a tee box or donates a raffle
prize you will be enriching the lives and school experience for the students
at the Dr. Leroy Mayo Elementary School.
