Can my child take medication at school?

Medications must be brought to the nurse in the original container with
pharmacy label.  Expiration dates must be effective.  Parents must complete a
parent request for dispensing medicine in school for for each medication to be
administered and prescription medication forms must be completed and signed by
the healthcare provider for each prescription medication to be administered.

Where can I obtain the appropriate medication forms?

Forms are available on this website under photos/docs or in the school/nurse's
office.  Please remember
to bring them to doctor visits.

What are the symptoms of H1N1 influenza?

The symptoms of H1N1 are the same as for other types of flu and include fever
(100 degrees or higher), cough, sore throat, muscle aches, fatigue and
sometimes vomiting and diarrhea.
