Please remember to schedule an annual physical for your child as they reach
their 10th birthday. The state is requiring that your child have a tetanus
booster and the meningococcal vaccine before they enter grade 6. No special
form is required, but please ask your pediatrician for a note that I can use
as proof of vaccination. If you have any questions please call me in the
