Medication Guidelines

If you child requires medication in school, please see the school nurse to obtain the proper documentation.  You must complete the "Authorization for Administration of Medications in School" form which includes a section to be completed by your child's physician, as well as your signature on the front and back of the form.   Medication may not be brought to school by any student. If you need to drop off medication either before or after school, please contact the Health Office (732-613-6853) and arrangements will be made to receive the medication before or after school hours. 
 
Asthma - Students with a diagnosis of asthma who will require treatment in school are also required to have the "Asthma Treatment Plan" form completed by the student's physician and signed by the parents.
 
Food Allergies - Students with severe food allergies  are requested to have 2 Epipens in school. In addition to the "Authorization for Administration of Medications in School" form, a complete "Food Allergy Action Plan". 
 
Epipens - Before you bring Epipens to school, please check the expiration date on the side of the box.  It would be greatly appreciated if you send in an Epipen that will not expire during the school year.  There are several staff members that have been specifically trained in Epipen use.  These staff members are considered "designees" and in the event the school nurse is unavailable, they are authorized to administer an Epipen if an emergency arises. Each student is assigned a designee in the event of an emergency.
