Intramural Sports

****Attention For the 2011-2012 School Year GYMNASTICS will be the only after
school sport that will be offered***

**CHECK ANNOUNCEMENT SECTION FOR DEADLINES WHEN REQUIRED FORMS ARE NEEDED BY**

Midland School offers several opportunities for students to participate in 
intramural or interschool sports. Below is a list of those sports, when they 
are played, and the grades that are allowed to participate.
     Volleyball: Fall, for 6th-8th grades only
     Gymnastics: Winter, for 4th-8th grades
     Indoor Soccer: Winter, for 4th-6th grades
    *Baseball: Spring, for 6th-8th grades only, tryouts in order to make the
               team; travel to other schools to play
    *Softball: Spring, for 6th-8th grades only, tryouts in order to make the 
               team; travel to other schools to play
PARENT RESPONSIBILITY:
In order for your child to participate in an after school sport, you must 
provide:
     1.A RECENT PHYSICAL:A student is to have a physical done by their private
      doctor. By state law, this physical must be done within 365 days prior 
      to the start of the first practice. For example, if the first day of 
      practice is to start on October 1, 2010, and your child's last physical
      was done on September 30, 2009, then a new physical would be needed
      because it will have been 366 days since the last physical. If the last
      physical was done October 2, 2009, then your child is OK to play because
      it has been 364 days since the last physical. The physical has 2 parts, 
      PART A is to be completed by the parent/guardian and PART B is to be 
      done by the doctor. Fill out PART A before seeing the physician so 
      he/she can review it when the physical is done. DO NOT SEPARATE THE 2
      PARTS!
   **Keep in mind that it may take several weeks to get an appointment for a 
   physical. If you think your child may be interested in playing any of the
   sports during the school year, and it has been awhile since they've had a
   physical done, it may be a good idea to schedule the physical during the  
   summer. That way they are covered for the whole school year. BY LAW, IF 
   YOU DO NOT HAVE A PHYSICAL BY THE FIRST DAY OF PRACTICE, YOUR CHILD WILL 
   NOT BE ABLE TO PARTICIPATE!!
     2. SUPPLEMENTAL HEALTH HISTORY FORM: If your child has had a recent
      physical, but it was done MORE THAN 60 DAYS prior to the first practice
      then you need to complete a SUPPLEMENTAL HEALTH HISTORY form. This form
      does not need to be filled out by the physician, just the parent.
     3. PARENT/GUARDIAN PERMISSION: If your child is interested in playing 
      more than one sport, you only need to complete one parent permission
      form and list all of the sports that your child wants to play.

   and they are listed as Athletic Participation Forms (parent consent, 
   physical, and health supplement).
STUDENT RESPONSIBILITY:
About 8-10 weeks prior to the start of one of the sports, a daily announcement
will be made at school reminding the students that if they are interested in 
participating in the after school sport, they need to stop by the nurse's 
office to SIGN UP for the sport and PICK UP THE REQUIRED FORMS. If a student 
waits until the last minute to sign up, they may not be able to participate 
if the required forms are not in by the first day of practice.
