Here you will find forms needed for athletes to partipate in sports, and for
volunteers to help out with our athletic program.
ATHLETIC FORMS
Physical must be on file with the school, and is active for one year from the date
of the physical. Physical must be filed at least one week prior to be allowed to
participate in any activity.
MEDICAL CONSENT FORMS
Medical Forms must be filled out and signed by a parent / guardian at least one
week prior to participation in any sport or activity. This means a 3 sport
athlete will need 3 of these forms throughout the year.
CONCUSSION FORM
The concussion form is an NJSIAA requirement, and must be on file with the school
prior to participation in any athletic contest. These forms are good for the
entire school year.
PROTECTING GOD'S CHILDREN PAPERWORK
All volunteers and staff of parishes of the Archdiocese of Newark must complete
Protecting God's Children. The process begins with a background check and
application, which takes up to a week to process. Once these are processed, staff
members must attend a Protecting God's Children course. Only after these criteria
have been met, can someone begin to help or volunteer at the school. All the
