MEDICATION INFORMATION:

As a general rule, a nurse will not accompany a field trip unless 
there is a student in attendance with a significant medical issue or 
a student who will need medication(either prescription or over the 
counter) dispensed during the trip.

If a student will need medication either for a school day trip or an 
overnight field trip, the parent should contact the school nurse as 
soon as possible to make arrangements.  The nurse can supply you 
with the district Request for Medication form or you may download it 
from this web page or from the district website at
www.spfk12.org/medication_form.pdf

Prior to the trip you will need to send the nurse the request for 
medication form and the medication in its original pharmacy 
container or over the counter packaging.
