SPORTS PHYSICALS: Forms are due in the Middle School Health Office no later
than Friday,September 11. Forms are available in the Middle School Office and
on my web page. Email or call me with any questions.
NEW VACCINATIONS REQUIRED FOR 6TH GRADE: Children born after January 1, 1997
and enrolled in grade six or transferring into a New Jersey school from
another state or country will be required to receive a booster dose of the
diptheria, tetanus and pertussis vaccine as well as one dose of meningococcal
vaccine.
PLEASE SEND YOUR CHILD'S VACCINATION FORMS TO THE HEALTH OFFICE.

provided to students under the age of 12. Information is available at
