Dear Parent/Guardian,
GUIDELINES FOR COMPLETING PAPERWORK FOR All SPORTS:
* A sports physical (NJDOE Annual Athletic Pre-Participation Physical
Examination Form, N.J.A.C.6A:16-2.2(h))MUST HAVE ALL AREAS of parts A & B
COMPLETELY filled out and on the School Nurses's desk by the FIRST WEEK
IN AUGUST, per WEEHAWKEN H.S. ATHLETICS POLICY.
If there is an insurance issue, please see our Team Physician,
Dr. Jesus Alfonso, 7907 Kennedy Blvd., North Bergen, NJ, 07047,
201-330-8747. He is contracted by the Weehawken BOE to do our athletic
physical forms free of charge to Weehawken student athletes.
* IF ANY PART OF THE FORM IS INCOMPLETE (parts A and/or B) IT WILL BE
RETURNED TO PARENT/GUARDIAN TO HAVE PHYSICIAN AND/OR PARENT/GUARDIAN
TO COMPLETE.
* EVERY private sport physical submitted to the school nurse, MUST be
REVIEWED & CLEARED by the SCHOOL PHYSICIAN, according to N.J.A.C.
6a:16-2.2.
* The Physical Forms CAN NOT be dated more than 60 days
prior to the starting date of the sport.
* In addition to the sport physical, each student will need the NJSIAA
Steroid Testing Policy, Consent for Random Testing signed and dated
by the student and the Parent/Guardian. Also, the Parent/Guardian and
Student will need to sign and date the NJSIAA Concussion Policy
Acknowledgement Form. Make sure you receive the handout on Sudden
Cardiac Death attached to the athletic physical, it is a mandate from
the NJDOE.
* Please read and review the MANDATORY NJDOE handout on Sudden Cardiac Death
and sign the form that stipulates that you read and reviewed article.
PLEASE CHECK THE SPORTS TEAMS STARTING DATES, to make sure your child's sports
physical is completely filled out, dated, signed and given into the School
Nurse's Office by said date. If not they will NOT be able to start with their
team mates. All regulations and policies need to be followed so the
Weehawken Board of Education and our Health Office is in compliance, and most
important to protect the health and care of all our student athletes.
