In addition to progress in all of the six major competencies of (1) medical
knowledge, (2) interpersonal and communication skills, (3) patient care,
(4) professionalism, (5)practice-based learning and improvement, and
(6)systems-based practice, there are specific tasks which must be
accomplished in each of the PGY years and which in aggregate comprise
a "portfolio" of documentation. Completion of these tasks is an important
criteria that the Committee on Clinical Competence uses to recommend
promotion and/or eligibility to sit for the ABIM certification exam. These
tasks are outlined as follows:
PGY-1:
Obtain and start reading a textbook of medicine (Harrison's is recommended)
Join the ACP-ASIM
Purchase MKSAP and start to read
Start reading one medical journal (such as NEJM, AIM, or AJM)
Start reading The Medical Letter
Learn to access and use PubMed (NLM), UpToDate, Ovid
Participate in online educational activities, such as the Johns Hopkins
Ambulatory Care Curriculum
Attend at least 60% of the Core Lectures (noontime and grand rounds)
Learn to use a clinical resource text such as The Washington Manual
Sit for the In Training Exam in October
Participate in one "major" CEX
Participate in two "mini-CEXs" in the continuity clinic
Participate in two "mini-CEXs" on the inpatient service
Take Cultural Competency/Communication Skills quiz
Keep a list of all patients, in a HIPPA compliant manner, seen in both the
inpatient and continuity clinic. Such a list is most useful when in a
password protected PDA. It should include major diagnoses and other
information helpful to provide care. This data is also used to document
exposure to a broad array of clinical problems.
Meet with their faculty adviser twice a year
Mentor a medical student
Start to consider a Research Project

Type a paper on how to reduce errors
Complete a take home quiz on ICD and CPT coding

PGY-3
Complete MKSAP
Pass USMLE Step 3 if not done previously
Type a review of a medication reviewed by the Medical Letter and compare
the review to the information from a pharmaceutical representative
Give a noon-time conference, with handouts and references, on one of the
following: EBM, medical statistics, domestic violence, cultural competency,
error reduction, IOM, NCQA, HIPPA, JCAHO, OSHA, ICD coding, CPT coding,
history of Medicare, history of Medicaid, history of the AMA, history of
the ACP-ASIM, office overhead,
Participate in online educational activities, such as the Johns Hopkins
Ambulatory Care Curriculum and ACPonline.org Clinical Problem Solving Cases
Attend at least 60% of the Core Lectures (noontime and grand rounds) and
summarize at least one GR for distribution
Present Ambulatory topic during block ambulatory rotation
Complete Curriculum, including Consultation Curriculum and
present a consultation topic
Sit for the In Training Exam in October
Participate in one "major" CEX
Participate in two "mini-CEXs" in the continuity clinic
Participate as a "patient" in an OSCE
Take a Cultural Competency/Communication Skills quiz
Analyze list of all patients, in a HIPPA compliant manner, seen in both the
inpatient and continuity clinic.
Meet with their faculty adviser twice a year
Mentor a PGY-1
Turn in Research Project Summary by first week of PGY-3 year. This must
include: Faculty Supervisor, Title, Background and Rationale for doing the
project, Methods, Materials, Expected Findings, and at least 3 references
(explanation from faculty adviser if such references are not available)
Complete project prior to graduation
Participate in a Community Service Activity
Attend talks on practice management, finanacial planning
Type a paper on the relation between pharmaceutical companies, ethics,
and professionalism
