Knowledge, skills, attitudes exhibited by residents, on behalf of the
patient, to demonstrate competency in following six general domains:
1. Medical Knowledge
Residents must demonstrate
[ ] knowledge of basic, clinical, and social sciences
including
[ ] knowledge of pathophysiology & mechanisms of disease
[ ] the ability to apply medical knowledge to clinical reasoning
[ ] motivation and self-direction to stay abreast of evolving
biomedical knowledge
2. Interpersonal and Communication Skills
Residents must demonstrate
[ ] use of open-ended, non-directive interviewing
[ ] use of empathetic interviewing
[ ] use of culturally sensitive interviewing
[ ] use of interviewing which is sensitive to diversity
[ ] ability to interview patients who do not communicate
in English
[ ] establishment of therapeutic, patient-centered, relationships
[ ] ability to work effectively with others as a member (or a
team leader) of the health care team including the
family, nurses, social workers, attendings.
[ ] ability to write legible, appropriate, problem-oriented
admission, progress, transfer, consult-request or
or consultation notes, (and discharge notes).
[ ] ability to counsel patients about health promotion,
disease prevention, smoking cessation
[ ] ability to communicate with patients from adolesence to
old age, including limitation of care and end-of-life
issues.
[ ] ability to communicate with patients about
domestic violence, alcohol and other addictions.
[ ] ability to communicate with "non-standard" patients such
as those who might be hostile.
3. Patient Care
Residents must demonstrate
[ ] ability to develop and carry out patient management plans
(including estimate of acuity of illness)
[ ] based on Clinical Judgment that appropriately synthesizes
[ ] information from accurate data gathering of essential
information;
[ ] information from physical examination skills
[ ] information from procedural skills
[ ] information from interpretation of laboratory studies
[ ] daily personal assessment of patients in the hospital
[ ] ability to provide longitudinal continuity of care in clinic
[ ] acute care in the ER and ICU
[ ] consultative care
4. Professionalism
Residents must demonstrate
[ ] knowledge of concept of self-regulating professions
and context of professionalism
[ ] knowledge of Fundamental Principles and Responsibilities
described in the ABIM "Physician Charter"
[ ] ability to create a portfolio that indicates strategies for
self-assessment and self-reflection
[ ] accountability to peers, nurses, attendings, patients
[ ] altruism toward community, peers
[ ] axcellence indicated by ongoing professional development
[ ] ability to recognize undue stress in self and peers
and knowledge of strategies and resources to reduce stress
[ ] aumanistic qualities towards patients of
compassion, empathy, and respect
[ ] ability to maintain trust by managing conflicts of interest
[ ] knowledge of concept of the "hidden curriculum"
[ ] ethical behavior and knowledge of medical ethics
5. Practice-based Learning and Improvement
Residents must
[ ] demonstrate a desire for formative feedback
[ ] participate in QI activities such as M&M conferences,
journal clubs, CPCs, etc
[ ] demonstrate strategies to reduce medical risks, reduce
errors and the ability to learn from mistakes
[ ] write accurate, legible, appropriate prescriptions in Clinic
[ ] review autopsy findings to understand illness
[ ] demonstrate methods to improve patient and physician safety
[ ] demonstrate methods to decrease nosocomial infections
[ ] respect patient rights
[ ] maintain patient confidentiality
[ ] keeps a patient log
[ ] analyze their patient population and
perform practice-based quality improvement activities
[ ] utilize information technology to support efficient
evidence-based medical care and consultations
[ ] use new patient-care medical problems as an
opportunity to learn rather than just call a consult
[ ] Know when to apply expert consensus and/or evidence-based
population guidelines to a specific patient and when
to use "common sense"
6. Systems-based Practice
Residents must
[ ] know different medical care delivery systems (managed and
private) available to provide healthcare:
advantanges, disadvantages and impact on patient and
physician
[ ] appreciate the relationship between the Social History and the
System
[ ] utilize clinical practice guidelines when appropriate to
provide cost-effective care for a population of patients
[ ] recognize and surrmount obstacles to appropriate patient care
[ ] effectively utilize hospital resources to achieve patient care
with awareness of costs
[ ] utilize community and clinic resources to advocate for successful care
[ ] understand how discharge notes and planning are a major
aspect of Systems-based practice, as segue, including:
[ ] writing an accurate Discharge Summary
[ ] writing a useful Discharge Note for the patient
[ ] striving for continuity of care
[ ] writing accurate discharge prescriptions
[ ] be familiar with other organizations that are part of the system:
[ ] NCQA [ ] OSHA [ ] HCFA/CMS [ ] OIG
