What should I do if I lose the practice chart?

piece of paper, note paper, index card, or postit as long as it has ALL of
the required information (dates, minutes, parent's signature and student's
name).

My instrument LOOKS broken........

Bring it to me RIGHT AWAY (even if it's not your regular lesson day!). It
just might need an adjustment. If it needs to be repaired, I will tell you
exactly what to tell the rental shop.
