All 7th and 8th grade students will be given the opportunity to participate in the after school modified sports program provided they meet the criteria set forth in the Middle School Handbook. In addition 8th graders will have the opportunity to play High School varsity sports as they become available. Please view the "Modified Sports Program" link on the MESMS website for detailed information and a parent informational sports letter from Mrs. Grimm each season.
Briefly, there are three sport seasons during the school year. Each season requires a physical covering the entire full season, a Health History Parent Guardian Permission Form, and a athletic release card (white card) signed by both the parent and student. They may be signed,dated, and completed only 30 DAYS PRIOR to the start of the season, no sooner.
Forms may be picked up in the Health office or downloaded off of this web site under "Download Important Documents". A 4 part white card must be picked up in the Health Office. All sports forms must be signed by the middle school nurse before participation can occur. Students recommended for High School sports must fill out one extra packet available in the health office only upon Athletics Director Fio Nardone's verification to the MESMS nurse.
A school Physician will be available for those students unable to get physicals on time or for physicals that are expiring. Please set up an appointment with the health office at least two weeks in advance of the physician's arrival date. Those students requiring physicals must hand in their completed packets to the Health Office two weeks prior to the start of the sport season they are playing to guarantee a physical. The entire completed packet must be handed in to the nurse no later then one full week prior to the start of the season.
Each sport will be announced on the web and in school. A letter will also be posted outside the Health Office prior to each sport season. Remember completed forms and physicals must be sent in on time prior to the start of the season in order to assure participation at the start of the season.
