of the required forms for athletics. The 2 forms you will need are the
"Participation Requirements/Emergency Procedure" and the "Physical Form".
PLEASE BE SURE THAT THE "DATE OF EXAM"
PORTION OF THE PHYSICAL IS FILLED OUT BEFORE TURNING IT IN
