Spring Sports Physicals
Physical forms must be completed, signed and returned to the nurses office by February 15. These forms are for physicals to be done by our school physician.
Re-certifications must be done by February 12. Please see the nurse for further information.
Physician forms completed by private physicians must be returned to the nurse by February 24
