Navigating the Maze of Transition
"Pathfinder" helps families and agencies find useful disability services and
resources. Staff can help solve especially unique and difficult issues faced
by people with disabilities and their families. They have first hand
experience working with people with disabilities and navigating disability
services. Search a database containing up-to-date information on over 4,000
programs and 1,700 agencies located within or serving Bristol, Tennessee.

employment, post-secondary education and/or training, independent
living, and community participation for future success. This
printable packet will help you prepare for the IEP meeting, and
even provides a student/parent questionnaire for transition
planning.

Students, it's your life!!! Learn about how to prepare for your
transition IEP meeting in the spring. Print and complete
the "Student Transition Assessment." Once completed, bring the
assessment form to Miss Powell for discussion, interpretation,
and guidance.
