| Registration Requirements
Out-of-State, Home School, or Private School Enrollees Must Have:
- Certified copy of Birth Certificate (not Mother’s Copy)
- Copy of Social Security card
- Official Tennessee immunization record. (If they don’t they will
need to take their copy to the Health Dept. and have it transferred.)
- Proof of Residency in the Central Elementary School zone
- Official custody papers bearing notary seal (if applicable)
In-County Enrollees Must Have:
- An Official Withdrawal Form (If not, they need to return to that
school and officially withdraw)
- Proof of Residency in Central Elementary School zone
- Official custody papers bearing notary seal (if applicable)
In-State Enrollees:
- Withdrawal form
- Proof of Residency in Central Elementary School zone
- Official custody papers bearing notary seal (if applicable)
