The 2009-2010 school year is rapidly coming to an end. All prescription and over-the-counter medications along with any supplies need to be picked up by the parent/guardian or a designee on or before Friday, May 28, 2010. After this date, the medications will be discarded, unless other arrangements have been made. Students CANNOT take their medication home with them.
Nothing will be stored over the summer.
A new physician form and/or over-the-counter form will need to be filled out for the 2010-2011 school year. Also, a new supply of medications will need to be brought in by the parent/guardian or designee. (these forms can be printed off by clicking on the medication form link).
