Updated Immunization Requirements to Enter 7^th Grade

Students are required to have the following updated immunizations before entering 7^th grade:

       1 dose of TdaP (if an immunization containing tetanus has not been received in the last 5 years)

       2 doses of varicella vaccine if a student has not had the chicken pox disease

       1 dose of meningococcal vaccine

If your student has received these immunizations, send an updated shot record from the physician or clinic to the school. School immunization records must be complete before schedules, locker assignments, or books will be given out in August.

ALL STUDENTS WHO NEED EPI PENS---
Due to new district policy, the clinic will no longer have a stock epi-pen.  You will need to supply one to the clinic.  Please make sure it has the Rx attached to the pen or box.
