<P align=center><SPAN class=c1> <SPAN style="FONT-SIZE: 24pt"><BR></SPAN><SPAN style="FONT-SIZE: 10pt"><SPAN style="FONT-SIZE: 12pt">Updated Immunization Requirements to Enter 7<SUP>th</SUP> Grade</P> <P class="MsoNormal c3"><SPAN class=c2><o:p></o:p></SPAN><SPAN class=c2><SPAN style="FONT-SIZE: 10pt"><SPAN style="FONT-SIZE: 12pt">Students are required to have the following updated immunizations before entering 7<SUP>th</SUP> grade:<o:p></o:p></SPAN></SPAN></SPAN></P> <P class="MsoListParagraphCxSpFirst c7"><SPAN style="FONT-SIZE: 10pt"><SPAN style="FONT-SIZE: 12pt"><SPAN class=c5><SPAN class=c4> 1</SPAN></SPAN><SPAN class=c2> dose of TdaP</SPAN> <SPAN class=c6>(if an immunization containing tetanus has not been received in the last 5 years)<o:p></o:p></SPAN></SPAN></SPAN></P> <P class="MsoListParagraphCxSpMiddle c7"><SPAN class=c2><SPAN style="FONT-SIZE: 10pt"><SPAN style="FONT-SIZE: 12pt"> 2 doses of varicella vaccine if a student has not had the chicken pox disease<o:p></o:p></SPAN></SPAN></SPAN></P> <P class="MsoListParagraphCxSpLast c9"><SPAN class=c2><SPAN style="FONT-SIZE: 10pt"><SPAN style="FONT-SIZE: 12pt"> 1 dose of meningococcal vaccine<o:p></o:p></SPAN></SPAN></SPAN></P> <P class="MsoNormal c10"><SPAN class=c2><o:p></o:p></SPAN></P> <P class="MsoNormal c10"><SPAN class=c2><SPAN style="FONT-SIZE: 10pt"><SPAN style="FONT-SIZE: 12pt">If your student has received these immunizations, send an updated shot record from the physician or clinic to the school.<SPAN class=c11> </SPAN><STRONG class=c13>School immunization records must be complete <SPAN class=c12>before</SPAN> schedules, locker assignments, or books will be given out in August.</STRONG> <o:p></o:p></SPAN></SPAN></SPAN></P></SPAN></SPAN></SPAN> <P class="MsoNormal c3"><SPAN class=c2><o:p></o:p></SPAN></P> <P class="MsoNormal c3"><BR><STRONG>ALL STUDENTS WHO NEED EPI PENS---<BR></STRONG>Due to new district policy, the clinic will no longer have a stock epi-pen. You will need to supply one to the clinic. Please make sure it has the Rx attached to the pen or box.<BR><BR></P>
