Where can I pick up the physical form for my child

The physical form may be downloaded from a link on the 
Sports Homepage at the stpiusv.info website.  If you do not 
have access to a computer or printer, the school nurse (Ms. Soto) 
will provide you a form.

When do I contact the office concerning Sports questions, issues, concerns, cancellations, etc...?

If the information you need is not on the websites' sport pages, 
you may contact me by calling the school administrative office at 
713 472-5172 and they will transfer your call to me or through 
email at rflores@stpiusv.info. If I am not available, please 
contact the adminstrative office for information.  My goal is to 
keep all of you well informed at all times. Thank you for your
