What is the protocol for sending medications to school?

All medication must be in the original container with expiration 
date and dosage instructions clearly visible. All medication is
to remain in the Nurses office and administered by the Nurse.

Before a student can receive medication, a permission form must
be signed by a parent or legal guardian.
If medication is to continue on a daily basis for a
month or longer, a physician signature is also required.

Where can I get a permission form?

On my website,go to Medication Forms and click on 
the link. Please note there are 2 pages. Please complete the
permisssion form and read the RRISD protocal on medications at
school. Copies are also available in my office,Room 205

What permission form do I need if my child requires an inhaler?

This is a two fold answer.
1. If you would like for the student to keep the inhaler in the
Nurse's office, A Request to Administer Medication form needs to
be completed and signed by a parent or guardian.
Should the student require the inhaler for more than ten
consecutive days,the form must be signed by a Physician.

2. If a parent or guardian would like for the student to carry
the inhaler during school hours, an additional form (Self-
Directed Treatment)must be completed by both a parent and
physician.

The form may be downloaded from the Home page of the RRISD
website or my website under Medication Forms.

What are the immunization requirements to enter school?

First Grade – Sixth Grade
5 doses of ^DTP, DTaP, DT, Td, Tdap with one on/after 4th
birthday OR 4 doses if one dose is on/after the 4th birthday
4 doses of ^°Polio with one on/after 4th birthday OR 3 doses if
one dose is on/after 4th birthday
2 doses of Measles, 1 dose Mumps and 1 dose Rubella on/after 1st
birthday
3 doses of ☼Hepatitis B
1 dose of Varicella on/after 1st birthday (if the child has NOT
had chickenpox)


Ages 7 years and older, 3 doses of DTP containing vaccine.
Seventh Grade
3 doses of ^DTP, DTaP, DT, Td, Tdap with one on/after 4th
birthday, one dose of Tdap is required within the last 5 years.
Td is acceptable in lieu of Tdap if a contraindication to
pertussis exists.
4 doses of ^°Polio with one on/after 4th birthday OR 3 doses if
one dose is on/after 4th birthday
2 doses of Measles, 1 dose Mumps and 1 dose Rubella on/after 1st
birthday
3 doses of ☼Hepatitis B
2 doses of #Varicella on/after 1st birthday (if the child has NOT
had chickenpox)
1 dose of Meningococcal


Eighth Grade – Twelfth Grade
3 doses of ^DTP, DTaP, DT, Td, Tdap with one on/after 4th
birthday, one dose of Tdap is required within the last 10 years.
Td is acceptable in lieu of Tdap if a contraindication to
pertussis exists.
4 doses of ^°Polio♦ with one on/after 4th birthday OR 3 doses if
one dose is on/after 4th birthday
2 doses of Measles, 1 dose Mumps and 1 dose Rubella on/after the
1st birthday
3 doses of ☼Hepatitis B
1 dose of #Varicella on/after 1st birthday (if the child has NOT
had chickenpox)
