Students ENTERING 7TH Grade 2011/2012
The following is a new list of State required immunizations:
•Tdap (if an immunization containing tetanus has not been received in
the last 5 years)
•2 doses of varicella vaccine if a student has not had chicken pox
•1 dose of meningococcal vaccine
If your child has received the required vaccinations, please provide
a physician's copy to the School Nursing office.
If your child needs the immunization, contact your Physician or Health
Care provider. Once the vaccine has been received, send or fax a copy of
documentation to school.
If you are electing not to have your child immunized, the appropriate
paperwork must be obtained and submitted prior to the beginning of school.
