Screening/Evaluation Process





  1. What do I do if I want my child to be screened for speech therapy or any other special education services?
  2. What happens on the day of the screening?



What do I do if I want my child to be screened for speech therapy or any other special education services?

If you feel that your child has a possible need for a special education
program, with or without the need for speech therapy, please:

1. Call Sandra Miller at the Special Education Annex, 281-482-0687,
ext. 6680, or email to smiller@fisdk12.net.

2. Tell Ms. Miller:
a. Your name, address, phone numbers, and email (if applicable)
b. Your child's name and birthdate
c. Your concerns

3. Ms. Miller will mail out a screening packet which includes:
a. Cover letter
b. Consent to screen
c. Parent Information (includes health history)
d. Parent Questionnaire for social and self-help skills

4. Return the completed packet to Ms. Miller at the Special
Education Annex, FISD, 402 Laurel, Friendswood, TX 77546

5. Ms. Miller will call you to set up a screening date.

6. Take your child to the nurse at Westwood, Kathleen Jolibois,
for a hearing/vision screening. Her number is 281-482-3341.

What happens on the day of the screening?

On the actual screening date, the SLP (Speech/Language Pathologist) and the
LSSP (Licensed Specialist in School Psychology) will use a screening protocol
and some play activities to assist in determining if a full evaluation is
needed. If an evaluation is recommended, we will discuss your rights as a
parent (Procedural Safeguards), and you will be given an opportunity to give
your consent to do the evaluation. We will set a date for the evaluation at
that time.
