Frequently Asked Questions: This page contains answers to common questions of
students and parents.

What if my child is sick or injured and can't participate?

All students that are excused ( they must have a note to be excused) 
must complete a Health assignment for a participation grade, ex. read
Chapter 1 answer the five review questions. Students that are out for a
long period of time will be assigned a book report on something of
interest to them. ex. "History of Baseball".

What if my child is injured but capable of participating?

The excuse note must say to the affect that your child should limit their
activity due to illness or minor injury. If a student is on limited activity
they are responsible for doing or choosing what activity or exercises they can do.

My child is in gymnastics, can she get a waiver from Wellness?

All waivers must be approved through the FISD administration, certain 
criteria must be met, once a child is approved for a waiver they are
excused. Grades will be given by the coach or trainer of the individual.All
students including
students with waivers still must take the state mandated FITNESSGRAM test
typically done before spring break.

My child suffers from asthma, can he carry his inhaler with him to gym?

No, all students that use an inhaler are required to store it with 
the school nurse. This is for the safety all students. (Of course there are
always exceptions contact the
school nurse in these cases)

Is the mile run required?

Yes, part of our curriculum includes fitness testing of our students. We use
the FitnessGram test mandated by TEA.

Why must my child do fitness articles in PE class?

The state of Texas has placed more emphasis on health and fitness. Since these
changes have taken place our district has changed the Wellness curriculum to
incorporate health ideas into our curriculum. Fitness articles along with
textbooks are
incorporated into the class. This is also a great way to include cross curriculum
writing into all areas of the educational setting. Our goal is to help
students read and
interpret various Wellness topics to help improve their own well being.

FHS PHYSICALS FAQ



1) When are physicals?
a. Physicals will be held Saturday May 15th at FHS in the Field House from
8:30 am � 2:30 pm.
2) Who needs a physical?
a. Any student that may participate in or tryout for any athletics both at the
High School or Junior
High including � Drill, Cheerleading, Lacrosse, Water Polo, Gymnastics.
3) How much are the physicals at FHS?
a. They are $25 with a portion of the money going back to the FHS Sports
Medicine Program.
4) Do I need to call and schedule a time?
a. No, they are done on a first come, first serve basis.
5) When is the best time to get there?
a. We are usually really busy at 8:30 am when the doors open and during the
lunch hours. The rest
of the day usually remains steady.
6) That is the Saturday before Memorial Day. Do you know that?
a. Yes, we are aware that physicals are scheduled for the Saturday before
Memorial Day. This was the
first available Saturday in May.
7) I heard that there will be echocardiograms offered. Is this true?
a. Yes. We have offered echocardiograms at our physicals for the past 10
years. They will be done on
site for an additional fee.
8) I heard ImPact testing will be offered. What is this?
a. In conjunction with the CHRISTUS St. John Sports Medicine Concussion
Center, our high school
athletes have the opportunity to take the ONLINE ImPact concussion baseline
test. This is a
computerized cognitive test that is used widely in professional hockey,
football and baseball
organizations. This baseline test will establish a pre-injury cognitive
baseline of the athletes that
choose to take this test. Should an athlete suffer a concussion, the athlete
can then be retested and
the results from both tests compared in order for their physician to better
assess when it is safe for
them to return to play.
b. This test is voluntary and free for all high school athletes. If any junior
high athlete wants to take
this test, there will be a $3 fee.
9) When will the forms be available to my son or daughter?
a. The 20010-11 Forms will be delivered to all schools (Bales, Windsong,
Junior High and the High
School) after May 1st.
10) I have an appointment with our doctor scheduled next week; can I have the
forms and have the
physical done then?
a. No. The forms are not available yet. They will be available and delivered
to campuses after May
1st.
11) Can I have a physical done prior to May 15th by my own physician?
a. We will not accept any physical form dated by the doctor prior to May 15th.
The physical is only
good for 1 calendar year. Should an athlete have a physical done prior to our
date and still be
participating in a sport when that physical expires, they will no longer be
able to continue to
participate.
b. Also, this physical is to clear our athletes to participate in athletics
for the 20010-11 School
Year.
12) Who are the doctors that do the physicals at FHS?
a. We use Dr. John and Dr. Bo Kirkwood and their staffs.
13) I have a form from a previous school year, can I use that?
a. No. UIL makes changes to the forms every year. These changes make forms
from previous years
invalid.
14) I have a conflict on May 15th and cannot make it to the physicals offered
by FHS, is there another
date that physicals will be done at school?
a. As of today, we do not have another date scheduled. If that should change
in the future, we will
post that information on the FISD Website.
15) When do I have to have the physicals done by?
a. Physicals must be completed and turned in to the FHS Athletic Training Room
before an athlete is
allowed to tryout, practice, participate or compete in any sport at FHS
including athletic periods.
Forms turned in after October 17, 2010 may not be accepted.
16) Where do I turn the physical forms in?
a. All physicals forms are to be turned in to the FHS Athletic Training Room.
There will be a drop box
located outside the Athletic Training Room during the summer or the forms can
be mailed. All of
that information is located on the front page of the physical packet.
17) Why is it recommended that I keep a copy of the forms?
a. Some of our athletes attend summer camps that require a copy of the
physical with their
registration. As the Athletic Training Room is not open during the summer
months, it may be
difficult to get a hold of a copy.
18) Can I fax the completed forms in?
a. No. Fax copies become difficult to read so we ask that the original forms
be turned in.
19) I did not get a copy of the blank physical forms, where can I get one?
a. Blank forms can be obtained at the Junior High, High School and
Administration building or you
can download them (after May 1st) from the Athletic page of the High School�s
website.
