Review the American Heart Association's Jump Rope for Heart Skills on-line.  Familiarize yourself with the written descriptions for each 'jump link' found on their webpage for the test (10/26).  The AHA's website is available in the Links Section.

* click on & print out the 'Health/PE Contract.'

* Review the information provided in this Contract with your parents.

* Sign & Date the Bottom Part of the Second Page.

* Enlist one of your parents to Sign & Date on the line under your information.

* Using Scissors, detach the bottom part of the second page (your signatures).

* Hang the Contract on your Fridge.

* Return the Bottom Part (the portion that you cut off of the contract, with your signature and your parent's signature on it) to me!!
