HEALTH INFORMATION for
New Students Entering Blake Middle School
Grades 6 - 8
REQUIREMENTS
Each new student to the Medfield school system must provide the following
before starting classes:
- Documentation of current immunizations, with dates
- Documentation of a recent physical (must be updated every year)
(If awaiting an appointment with a health care provider, submit the
completed form as soon as possible, generally within a month of starting
classes)
If new student is entering Grade 7:
Immunizations required by Massachusetts Department of Public Health:
- Hepatitis B vaccine (at least the first dose of the 2 or 3 dose series)
- Td (Tetanus) booster, if more than 5 years have elapsed since last dose
- Physician-certified reliable history of chicken pox (varicella)or evidence
of immunity OR 1-2 doses of Varivax vaccine (depending on child��s age)
- A second measles-containing vaccine (i.e., 2nd MMR)
If the student is interested in interscholastic sports:
A written MD or NP report of a physical examination, completed every 13
months, is required for all interscholastic sports (football, field hockey,
volleyball, basketball, baseball, softball, track and field). MIAA
regulations require the exam dated within the calendar year of the first day
of practice. The physical will expire 13 months after the date. In order for
the student to retain eligibility after that date, a new physical form needs
to be submitted.
