RESIDENTS BENEFITS SUMMARY

BENEFIT DESCRIPTION 

1.	Hospital Insurance

3.  Selected Benefits Which are Covered

           (1)  Psychological Support Services/Counseling
                Referral for appropriate services can be obtained     
                from the resident’s faculty advisor and/or the Director   
                of Psychiatry.
 
(a) 	in-patient                 100% covered charges for
	                           60 days per year                       
(b) 	out-patient                50% of covered charges 
                                   to a maxium of $50 per visit.
                                   $1,500 maximum per year  


            (2)  Alcohol or Drug Abuse Treatment
Referral for appropriate treatment can be obtained from the resident’s 
faculty advisor, the Director of Addiction Medicine, or the Director of 
Psychiatry.  The Medical Society of the State of New York also provides 
resources to assist physicians with alcohol and drug abuse problems.  The 
Hospital policy on Alcohol/Substance Abuse is included in the Orientation 
Manual. 
                
    (a)in-patient      100% covered charges for 30 days per year.
    (b)out-patient     80% of covered charges to a maximum of 60 
                       sessions each year up to 20 sessions may be 
                       used for family members.                    
                                                                           

             (3)  Physical Therapy/Occupational Therapy

(a) 	in-patient   100% covered charges. (In-patient or 
                                   a certified Home Health Care Agency)

                                                                          
(b) 	out-patient  80% covered charges. (Recertification
                                  required after 8 visits).             

4.  Dental Insurance  

                  a. Eligibility Date          Date of hire.

                  b. Deductible - Individual   $25.00 per calendar year.
                                  Family       $75.00 per calendar year.

                  c. Maximum Benefits          $1,000 annual maximum 
                                               per person per calendar
                                               year.
                                               This does not apply

5.  Life Insurance

9. Maternity Leave and Professional Leave of Absence for Residents

Residents will be provided with a maximum of two (2) months maternity leave 
per year; accrued sick time may be used to add to the (2)-month leave at full 
salary if the maternity leave/disability is extended.  The resident will be 
paid the Hospital disability rate for the duration of the maternity leave to 
a maximum of (6) months. 

Notwithstanding the above Hospital sponsored benefits, each Training Program 
and Specialty Board will have its own policies on the effect of the leave of 
absence on completion of training.  The American Board of Internal Medicine 
(ABIM) provides for up to (1) month of leave per year of training for 
maternity leave or illness (including pregnancy-related disabilities).  The 
ABIM requires that training be extended to make up any absences exceeding one 
month per year.  The ABIM also stipulates that vacation leave is essential 
and must not be forfeited to compensate for extended illness, late starts, 
maternity leave or other purposes/professional leave of absence.     

Requests for professional leave of absence should be made directly to the 
Program Director and will be considered on an individual basis.  The Hospital 
does not provide salary for a professional leave of absence unless related to 
a medical condition.  

10.  Paternity Leave

For the birth of a child, one (1) day is provided for the father.
                           

11.  Pension Plan

                                          service.                
     c.  Cost to Employee                 None.
     d.  Vesting                          100% after 5 years of service.
     e.	 Normal Retirement                Age 65.

 

12.   Sick Days   	                       

      Residents must immediately notify the Chief Medical Resident and 
      Medical                
      Education office of absence due to illness.  Within 24 hours, residents 
      must report to Employee Health Clinic, or the Emergency Room if
      Employee Health Clinic is closed.  Residents receive full pay for up
      to 12 sick days per year.
 
13.  Holidays/Vacation         

      Official hospital holidays are posted by the Department of Human
      Resources.                                                              
                                                         
      The Chief Medical Residents and the Program Director will determine 
      the work hours for those holidays. All residents are provided with 4
      weeks of paid vacation.  Vacation schedules are made up by the Chief
      Medical Residents who will attempt to accommodate specific requests
      made in writing sufficiently in advance.

14.  Voluntary Annuity Programs 

      All employees may participate in this Voluntary Tax Sheltered 
      Annuity (403-B)Program through salary deductions.  If you are interested
      in this program, or need further information, please contact the
      Benefits Coordinator.

15.  Municipal Credit Union   

     All employees may participate in this program.  If you are interested
     In this program, or need further information, please contact the 
     Benefit Coordinator.

16.  Supplemental Life Insurance  

     All employees may participate in this program.  If you are
     interested in this program, or need further information, please
     contact the Benefits Coordinator.

17. Meals    

     Meal cards are provided to residents to subsidize the cost of
     meals, up to $2.50 for breakfast and $4.50 for lunch and dinner.
     Medical Records Department makes the meal cards available to
     residents whose charts have been completed in a timely fashion.
     When residents are on-call in the evening, sandwiches are available
     in the Emergency Room Lounge. Vending machine are available in the
     cafeteria. 


18. Laundry              
         
     The Hospital provides 2 white coats per year, however, laundering is
     the responsibility of the resident.


19.  Duty Hours And Supervision
              
     No more than one night in three on call.  We follow New York State 
     supervision and work hour regulations as well as ACGME duty hour   
     regulations.

20.  Physical Examination/Medical  Support Services   

     The Employee Health Service provides a pre-employment physical 
     examination including appropriate vaccinations.  A physical examination 
     is also scheduled around the anniversary date of employment and is also 
     provided by the Hospital at no cost to the employee.
                              
21.  Malpractice Insurance          

     The Hospital is self-insured and covers the cost of professional 
     liability insurance for residents, and this includes “Tail” coverage.


22.   Live-In Accomodations 

     The hospital does not provide living accommodations.

23.   2003-2004 Salaries for Internal Medicine Residents

        PGY  I  :   $ 41,000
        PGY  II :   $ 43,500
        PGY III :   $ 46,500  
        

If the terms of this summary sheet and the plan document differ, the plan 
document will govern.  Your coverage may be terminated or modified in whole 
or in part under the terms and provisions of the plan document.

 QUESTIONS ABOUT MATERNITY LEAVE.


Q.  How long does Maternity Leave last?

A. 	Maternity Leave begins whenever the resident’s physician indicates
    she should no longer work. Generally, residents are given up to 
    two months of Maternity Leave at full pay.


Q. 	What if I need more than two months?

A. 	First, your accrued sick days are used - you get 12 of these per year.
    Next, short-term disability would come into effect and in the unlikely 
event
    that you are out over six months, long-term disability would become 
effective.


Q. 	What about my eligibility to sit for my Specialty Boards and my 
    promotion by my training program?

A. 	The American Board of Internal Medicine requires that there be 36
    months of training of which at least 24 months “hand on” medical 
    care.  Any “make-up time” required to fulfill the educational 
    requirements must be discussed with the Program Director.  


Q. 	While I’m put on Maternity Leave, who pays my Medical and Life 
Insurance Benefits?

A. 	Medical and Life Insurance Benefits continue to be paid for by the
    Hospital during the Leave of Absence.


Q. 	Are schedule accommodations allowed for pregnant residents?

A. 	Yes.  If you require an adjustment in your schedule, this should be 
discussed with the Chief Medical Resident and the Director of Medical 
Education.


Q.  What about Paternity Leave?

A.  For the birth of a child, one day is provided to the father.
