Make certain that the Medical Education Office (960-6202/6201/6211) has your 
beeper number so that you can be notified of consults.  You should always try 
to speak directly to the physician who called in the consultation to find out 
specifically what issues they want addressed.  The physician who called in 
the consult, should have filled out a consultation request sheet.  But even 
if they have not, that is the form on which you should write up your history, 
physical examination, lab data and assessment.  When I see the patient, I 
will sign your note and write my own notes in the Progress section of the 
medical record.  When you provide a Consultation, please keep the following 
in mind:
1.  Before even starting the history, you must make certain that you are not 
interrupting the patient and that the patient is as comfortable as possible.  
2.  Your history taking must start off open-ended, and must be empathetic.
3.  The Social History is extremely important.
4.  You will also need to obtain very specific information regarding the 
names of physicians who have previously taken care of this patient.
5.  Previous records from our Hospital must be reviewed.
6.  The physical examination should focus on the endocrine system.  Patients 
must be given a drink to examine the thyroid.  Patients with the potential 
for neuropathy should have an exam that evaluates that.

up every few days, you should ALWAYS ask to have him paged.  Confirm that the 
person who takes the message has your full name, your beeper number, and a 
phone number at which to reach you.

Residents who participate in the Endocrine Elective will have an opportunity 
to provide comments, suggestions, and feedback regarding the rotation at the 
end of the month.  They will also have a chance to receive feedback then as 
well.     

  
Rotation Outline

I.  Introduction:  Outcomes and Competency

    During the Endocrinology rotation, you will be exposed to common
    metabolic and hormonally mediated conditions.  A necessary
    outcome of the experience is that you become competent in the
    diagnosis and treatment of several of these hormonal conditions.
    Your competency will be evaluated in the six domains of:

    1.  Medical knowledge
    2.  Interpersonal and communication skills
    3.  Patient care
    4.  Professionalism
    5.  Practice-based learning and improvement
    6.  Systems-based practice

    You will have the opportunity to demonstrate your competency
    in these domains of Endocrinology during direct observation
    of your interaction with patients, your presentation of
    material you have researched, and as part of standarized

II. Specific Endocrinology topics with which you should develop
    competency

    1.  Diagnosis and treatment of Type 1 and Type 2 diabetes
    2.  Empathetic communication with patients who have diabetes
    3.  Cultural issues in diagnosis and treatment of diabetes
    4.  Gender and age issues in diagnosis and treatment of diabetes
    5.  Issues of professionalism in treating diabetes
    6.  Practice guidelines and quality improvement and
        error reduction when caring for the patient with diabetes
    7.  Evaluation of hypoglycemia, hypocalcemia, hypoadrenalism
    8.  Diagnosis and treatment of hyperthyroidism and
        hypothryoidism
    9.  Evaluation of thyroid nodules and treatment of
        thyroid cancer
   10.  Evaluation of amenorrhea, hirsutism, and infertility
   11.  Evaluation of impotence
   12.  Evaluation and treatment of hypercalcemia

III. References

     Read all sections of Harrison's Principles of Internal
     Medicine which apply to Endocrinology, as well as the
     Endocrine section of the current MKSAP.  Additional

 IV. Faculty and Supervision

     The Endocrine rotation is supervised by Dr. James Hellerman.
     Please contact him at Room 717 (x6202) when you start the
     rotation.  Make certain you also have his answering service
     number.  Discuss all consultations with him.

IV.  Schedule

     Check with the office regularly and ensure the office can
     readily contact you.  If you have other obligations, such as
     Continuity Clinic, interviews, etc, please make certain that
     all staff are aware of these commitments.

     Monday afternoon, you should attend Endocrine Private Practice
     at 2016 Bronxdale Avenue, Suite 302 (718-822-1515) from
     1:00 PM - 5:00 PM.

     Thursday at 8:00 AM you should attend Medical Grand Rounds,
     and Thursday afternoon from 1:30 PM to 5:00 PM you should
     attend Endocrine Clinic on the 5th floor of the Clinic Building.

     Most days, you should attend the scheduled Noon Time conferences.

V.  Evaluation

    Prior to the end of the rotation, you should sit down with
    the preceptor to go over your evaluation of competency as well
    as to provide your opinion as to the quality of the educational
    experience and any suggestions you might have to improve the
    rotation.
