New K Registration

To register for
T.V.J.A. Kindergarten Class 2008-2009
Please call the school at 503-649-5518 and make a
Developmental Skills Evaluation Appointment.

Children must be Five before September 1, 2008

Items needed:
	1) Registration papers completed and fees paid
	2) A copy of your child’s birth certificate
	3) Your child’s immunization record
	4) A reference letter and/or a progress report from your child’s 
	   previous child care provider(s)
