What does it mean to set up a paper correctly?

Your student must have their paper properly positioned with the heading on
the left. They must have their name, date and teacher's name in the right
corner and the title of the assignment in the middle, on the top line.
Filure to "set up" a paper correctly will result in points taken off the
total grade for that page.
