What does it mean to set up a paper correctly?

Your student must have their paper properly positioned with the holes
on the left. They must have their name, date and teacher's name on the top
right corner and the title of the assignment in the middle, on the top
line.
Failure to "set up" a paper correctly will
result in re-doing the assignment.
